This assessment is for you to honestly assess areas of your life in a safe space where nobody else knows your results of your assessment.

Please answer with the first choice on each question to have the most accurate outcome of the assessment.

Coachability Assessment

Please complete this coachability questionnaire prior to our first session. For each statement, choose a number from 1 to 5:
1 = Not like me at all     5 = Very true of me / describes me well.

Important: Lower scores in any area are not an indication that anything is wrong with you. They simply highlight where we might benefit from a first coaching session together — and that first session with Path2Clarity Coaching is always free.

Self Discovery Questions (SDQ)

Reflect on how much these statements describe your curiosity, self-awareness, and interest in learning about yourself.

1. I describe myself as someone who is curious about new things and explores opportunities for personal growth.
2. I have sought feedback or ideas from others toward my personal growth.
3. I understand the difference between coaching and counseling and see how each serves me best as I seek to grow and attain my personal goals.
4. I know the coaching relationship is about my thoughts and agenda, and I am open to becoming aware of new and different thoughts and potential actions.
5. I am not experiencing traumatic stress issues keeping me from fully functioning in my daily activities.
Goals and Actions

These questions focus on your belief in change, goal-setting, and willingness to engage in coaching conversations.

1. I believe people can change and grow into their best selves.
2. I want to grow my ability to identify personal or professional growth opportunities.
3. I am willing to be asked thought-provoking questions.
4. I appreciate being asked for my permission to have a tough question asked if it helps me get to deeper levels of thought.
5. I have been a goals-driven person in my life.
6. I understand that coaching leads to deeper levels of thought and setting goals leading to actions.
7. I am ready to begin setting new goals as of today.
8. I recognize I am not where I want to be in life and want to work towards my self-growth and setting new goals and actions.
9. I accept full responsibility for my thoughts and actions.
10. I am a reliable person who can agree to and keep commitments.
Adaptability

These items explore how you relate to change, relationships, and possibilities in your life.

1. I appreciate the differences in people and can validate opinions different than my own.
2. I have not sought opportunities for my self-development as much as I would like, and I want to focus on my self-development at this point of my life.
3. I realize I have areas of my life I want to change and work towards my best life balance.
4. I want to find my purpose and identify thoughts and actions for how I can move that way in my life.
5. I accept responsibility for my life and want to improve my patterns of thinking and behavior leading to an improved personal life and career.
6. I can describe “one good thing” when asked without having to go into deep thought to identify what is a good thing in my life.
7. I need to change my life and I’m willing to work to improve my thinking and actions.
8. I form and keep relationships easily.
9. I have many long-term friendships that bring me much joy in life.
10. I am aware of how personal interactions either drain or recharge me.
11. My family is very important to me, and I interact with them in very positive ways.
12. I feel challenged with certain relationships and wish to work toward better communication and outcomes.
13. I wake each day seeing opportunities for myself to grow.
14. I feel I can be myself and share my thoughts and ideas with most of my friends and family.
15. I am comfortable knowing my coach will not give me the answers but will be a guide as we journey through my thoughts together.
Actions

These questions look at your readiness to invest time, energy, and honesty in the coaching process.

1. I am willing to devote time and resources to my personal growth, with the eventual result being life-long personal improvement changes.
2. I am willing to identify and describe my reasons why something we discuss is meaningful and important to me.
3. I am willing to realize and receive feedback if a topic or discussion is difficult, while remaining present with my coach to work through my thoughts.
4. I realize we may identify and discuss my fears which may be uncomfortable, yet necessary for me to grow.
5. I realize that true meaningful growth comes at the edge of our comfort zones.
